Description

Job Title: Social Worker - Continuing Health Care (CHC) - 2 posts
Salary: £39,027 – £44,463 per annum (pro rata if part-time) 
Grade: E
Contract Type: Fixed Term - 24 months
Working Hours: 37 hours per week

Thurrock Council are looking for two qualified Social Workers to work within our Continuing Healthcare (CHC) Adult Social Care (ASC) Team under a fixed two-year contract. 

As a registered and qualified Social Worker, you will be working within a multi-disciplinary environment collaboratively with your social care colleagues and other professionals in the varied and complex field of NHS CHC. You will manage a diverse caseload, working across all care groups including older and younger adults, with mental health concerns and physical and learning disabilities.

About the Role 

The role involves working within Adult Services to undertake Care Act assessments and reviews for those with identified complex needs with potential eligibility for CHC. You will work from a multi-disciplinary perspective and work closely with health and social work colleagues to determine NHS eligibility, ensuring legal compliance (Care Act 2014, Mental Capacity Act) and promoting independence while supporting service users and families. 

What We’re Looking For 

You must, in the foremost, have knowledge and working experience of the National Framework for NHS Continuing Healthcare and NHS Funded Nursing Care alongside working within the duties and practice guidance of the Care Act 2014.   

We’re therefore looking for two qualified, registered Social Workers who have: 

  • Working knowledge and experience of the National Framework for NHS Continuing Healthcare and NHS Funded Nursing Care July 2022 (Revised) and the Care Act 2014.
  • Good advocacy skills 
  • A sound working knowledge and experience of working in CHC procedures and processes. 
  • A sound understanding of how to apply a strengths-based approach.
  • Understanding of social work issues, legal frameworks, and social services statutory duties.
  • Commitment and ability to demonstrate social work values and behaviours.
  • Good multi-disciplinary working skills 
  • Good communications skills 
  • Good experience in working with CHC processes. 
  • Good report writing skills. 

Essential qualifications: A Social Work qualification and be registered with Social Work England

DBS Check: At Enhanced Level with Adults Barred List
